CKV WAIT LIST FORM 2026

Families interested in enrolling their child at Catalina Kid Ventures are invited to complete this form and submit it either by:

• Dropping it off in person to CKV at: 430 Avalon Canyon Rd, Avalon 90704
• Postal mailing it to: Catalina Kid Ventures, P.O. Box 1899, Avalon 90704
• Scanning and emailing the completed form to: CatalinaKidVentures@icloud.com. Please include WAIT LIST in the memo/subject line of your email.  Thank you :)

• This form is required for all prescription and nonprescription medications. Items shall be submitted to CKV with the child's name and dated currently.
• Prescription and nonprescription medications must be stored in the original bottle/can with an unaltered label. Medications that require refrigeration will be stored properly.
• Prescriptions and nonprescription medications shall be administered in accordance with label directions.
• Written consent must be provided by the parent, permitting Catalina Kid Ventures personnel to administer medications to the child. Instructions shall not conflict with the prescription label or product label directions.
